Possible causes:

Allergic reactions can occur when your puppy comes into contact with an allergen or eats foods that are prone to cause allergies. This can lead to severe itchiness and constant ear-scratching. Under a doctor's guidance, anti-histamine medications can be taken to relieve the itchiness caused by allergies.

Ear mites:

Ear mites are parasites that live in the ear canal. They can cause increasing discharge and even abscesses, leading to intense itchiness. Your puppy will frequently scratch its ears. If not treated promptly, the condition may worsen. It's essential to maintain good local hygiene, bathe regularly, and follow the doctor's advice for antibiotic treatment.
